Incubation period can take six days or more.
Most people express no symptoms. But near the end of this period, people that are infected can transmit the virus to others. 20-50% of people infected are asymptomatic - that is, they do not appear to be sick. Some will assume anyone they encounter might feel fine, but in fact they could be infected and contagious.
For half of the infected, mild disease
Epithelial cells in the respiratory and the GI tract express ACE2. This means the virus can enter and spread throughout digestive and respiratory systems. Most report 4-8 days of infection before displaying symptoms of fever, fatigue, aches, anosmia, cough, sore throat, shortness of breath, and diarrhea.
For 5-10% of the infected, severe symptoms
2 weeks after infection, 5-10% progress to a severe disease phase with bilateral pneumonia requiring supplemental oxygen. About 2.% of patients progress to acute respiratory distress syndrome (ARDS) marked by alveolar breakdown and pulmonary edema. Some 1% of infections are fatal. Note that percentages are based on infections, not diagnosed cases.
What recovery looks like
About one-third of adults who tested positive for coronavirus said they hadn’t returned to normal health two to three weeks later. Eighty-seven percent of patients who had recovered from Covid-19 reported persistence of at least one symptom 60 days later, particularly fatigue and shortness of breath.
The Long Haul
The U.S. CDC says antibodies develop within one to three weeks after infection. But people who meet the criteria for recovery are testing positive for the virus again after three or more months. Re-tests of more than half of those people found no evidence of the live virus circulating.
